All forms should be returned, with the correct payment to: Hotel allocation will be made on a first come first served basis. You are advised to register early, the number of hotel rooms at the convention site is limited. The Organizing Secretariat is able to arrange for special accommodation in case of special needs such as the use of a wheelchair. Please do not contact the hotel directly as all reservations are handled by Hospitalité Québec. Registration fee paymentWe recommend that you make payments with your credit card. Bank transfers are expensive, since you will have to pay the bank charges twice: in your own country and in Canada. All payments should be made in advance in $US (payments in any other currency will not be accepted) and only by the following procedures:

Currency exchange charges or bank collection fees are not included in the total costs and are to be paid by the registrant. Currency exchange charges or bank collection fees are the sole responsibility of the registrant. Registration and accommodation are only complete if the form is submitted together with registration payment, and if the bank draft for the correct amount or the credit card information is approved. Registration forms may be faxed or mailed to the Organizing Secretariat.
Accompanying persons' registrationAccompanying persons are admitted to the Welcome reception, Farewell reception, wine and cheese, coffee breaks, all included meals and the exhibition. The registration fee does not include conference materials or admission to the scientific sessions.
SubstitutionsSubstitutions are welcome if you are unable to attend the Congress and would like to send another person in your place. There will be a $ 50 US charge. Please advise the Organizing Secretariat of your replacement. Substitutions will also be accepted on-site. Please note that only one person may attend per registration. You may not split a registration (i.e. the same person must attend the entire program on a single registration).
Important Notices
Collective PaymentIf a collective payment has been made, please indicate the name of the remitter and attach a list of participants in order to confirm accurately their individual payments.
Registration ConfirmationYou will receive a Confirmation Letter as soon as the Organizing Secretariat has received your payment. Allow approximately one week after receipt of your completed registration form and required payment to receive confirmation with details of your registration. An e-mail confirmation will be mailed if an e-mail address is provided on the registration form. Otherwise, your confirmation will be faxed or mailed to you. Upon arrival the Confirmation Letter must be presented at the Registration Desk if the account has been settled, or at the on-site Registration Desk if a balance is remaining. In order to avoid any problems when registering at the Congress, participants are kindly requested to carry with them the complete documentation of payment of the registration fee.
Cancellation/ RefundA refund of your Registration Fee less an administration charge of $50 US will only be available by providing notification of cancellation in writing (facsimile and e-mails are also accepted) to the Organizing Secretariat prior July 1st, 2003. For cancellations after July 1st, 2003 no refunds will be given. However, registrants will receive their Congress material by mail after the meeting. Refunds will be made after the conference. Please clearly indicate the bank, branch and account number, as well as the Swift code and full address of both the bank and the registered person to which the refund should be made. Any money transferring charges will be deducted from the registration fee.
On-Site RegistrationOn-site registration will start on September 7th, 2003. The on -site registration desk will be located in the Château Mont Tremblant and will be open from 08.00 until 17.30 all days. On-site payments can be made in cash ($US) or by one of the following credit cards: Visa, Master\Eurocard and American Express. It cannot be guaranteed that participants who register on-site will receive a delegate bag with the Final Program and other Congress Materials. Availability will depend on the number of on-site registrants.
Badges to access the conference siteYou will receive a personalized badge when collecting your registration documentation. This badge has to be worn at all times to enable you to access the scientific sessions and exhibition area.
Passport and Visa InformationAn Identity Card is sufficient for participants residing in the USA (although a passport is recommended). All other participants need a valid passport to enter Canada. Visas may be required for participants of some countries. For specific information, please contact the nearest Canadian Embassy at least six weeks prior to your departure for Canada.Credit card and Currency ExchangeMajor credit cards (VISA, American Express and MasterCard) are accepted in most shops, hotels and restaurants.The currency of Canada is the Canadian dollar. The following conversion table gives an idea of exchange rates as of September, 2002 (subject to fluctuations):
